RESOLUTION RESCINDING PRELIMINARY AND FINAL PLAT
FOR MULTICARE RESUBDIVISION
WHEREAS, the petitioner is the owner of certain property consisting of approximately 10
acres generally located at the southeast corner of McCarthy Road and Walker Road in Lemont,
Illinois and legally described in Exhibit "A" attached; and
WHEREAS, the Village Board approved a Plat of Resubdivision for the subject property,
on December 8, 1997 by Resolution No. R- 66 -97; and
WHEREAS the petitioner has made a request to the Village Board to rescind the Plat of
Resubdivision in order to expand the parking lot at the Multicare facility; and
WHEREAS, the petioner has agreed to submit a check in the amount of $10,500.00 to cover
previous land acquisition costs.
NOW, 1'HLREFORE BE IT RESOLVED by the President and Board of Trustees of the
Village of Lemont that the Plat for Multicare Resubdivision, is hereby rescinded.

THE VILLAGE OF LEMONT, COUNTIES OF COOK, WILL, AND DuPAGE, ILLINOIS,

I am writing to follow up on your letter regarding the proposed recision of the Multicare Plat of
Subdivision. After a review with the Village staff, it appears that we can support such a recision
based upon the following:
1. The land acquisition costs for Stelmokas roadway dedication are paid in full ($10,500.00).
Century Care does not object to a future dedication of Walker Road if it should be needed
for a future roadway extension through the Stelmokas property. This would likely be done
by Covington Knolls in the future.
If agreed toby Century Care, Village staff will forward a recommendation to the Village Board to
rescind the plat of subdivision. The parking lot improvement will be considered as part of the
amended site plan provided that all the landscaping and berming are completed as part of the parking
lot improvement.
To begin this process, I would request that a letter be submitted reiterating and agreeing to these
points. We should then receive a check in the amount of $10,500.00 for our costs for the land
acquisition. After this is submitted, staff will forward a recommendation to the Village Board to
rescind the plat. Once that is completed, you may apply for building permits for the parking lot
improvement.

Rescission of Plat approval and issuance of permit for parking lot expansion
Dear Scott:
This will confirm the arrangements for the implementation of the agreement outlined in your letter
of April 14, 1998, and our subsequent conversation. The final agreement is as follows:
1. Century Care Management, Inc., and agrees to the terms in you letter dated April 14,
1998, a copy of which is attached for your convenience;
2. Century Care Management, Inc., is making application for the permit to expand the
parking lot in accordance with the plans prepared by Tech 3 (the parking lot plans are
the same ones submitted with the subdivision application);
3. Century Care Management, Inc., will send me a check payable to the Village in the
amount of $10,500.00, and I will deliver the check the night the Village Board
rescinds plat approval and issues the permit for parking lot expansion.
We realize that the Board may not actually issue the permit, but the paper work can be completed
and the approved permit can be released on the day the Board votes to rescind the plat and the check
is delivered.
In the meantime, my client is working on the permit application, and we understand that the permit
process may take up to thirty days. When we have a date for permit approval, we will coordinate

with you on the meeting date at which time the Village Board may act and the check may be
delivered.
