RESOLUTION AUTHORIZING CONTRACT CHANGE ORDER FOR
CONSTRUCTION SERVICES RENDERED IN CONJUNCTION WITH THE
MC CARTHY ROAD SIDEWALK PROJECT — PHASE II
WHEREAS, the Village of Lemont has previously awarded a construction contract to Hook's
Concrete and Construction Co., Inc. for the McCarthy Road Sidewalk Project — Phase II; and
WHEREAS, the Village of Lemont has determined that certain changes to the contract
totaling $11,161.20 are necessary to satisfactorily complete the construction project; and
WHEREAS, the contract changes are a result of final quantities as measured in place and
changes that were not originally contemplated at the time the contract was awarded.
NOW, THEREFORE, BE IT RESOLVED, by the President and Board of Trustees that the
Contract Change Order with to Hook's Concrete and Construction Co., Inc in the amount of
$11,161.20 is hereby approved.
PASSED AND APPROVED BY THE PRESIDENT AND BOARD OF TRUSTEES OF
THE VILLAGE OF LEMONT, COUNTIES OF COOK, WILL AND DUPAGE,
ILLINOIS, on this 27th day of November , 2000.

This letter is to certify that Hook's Concrete & Construction Company, Inc. has
satisfactorily furnished labor and materials in accordance with the terms of their Contract,
and as outlined on the attached "Engineer's Payment Estimate No. 3 & Final ".
O The Contractor has completed all work on the project. The final project cost of
$73,063.95 is $11,161.20 (18 %) over the awarded Contract value of $61,902.75. The
reason for the extra work was the need to replace approximately 172 feet of deteriorated
15 -inch storm sewer that was discovered in the field during construction. Also included in
o the extra work is the necessary stone backfill and pavement repairs associated with the
sewer replacement work. (Without the culvert replacement, the sidewalk construction

would probably have settled and collapsed into the sewer in the near future.)

O Attached are three (3) copies of "Change Order No. 1 & Final ", which needs to be
o approved by the Village Board. Upon approval, please return one (1) signed copy to me for

We hereby recommend the payment of $14,144.57 as final payment to Hook's
Concrete & Construction Company, Inc., P.O. Box 492, Glenwood, IL. 60425, to be
paid upon receipt of the necessary "Final Waiver of Lien" with "Contractor's Affidavit ", and
0 matching invoice in the aforementioned amount.
