BE IT RESOVVED, by the Board of Trustees •

Village of Lemont , Illinois, that there is hereby

appropriated the sum of S. 210,000 of Motor Fuel Tax funds for the purpose of maintaining
streets and highways under the applicable provisions of the Illinois Highway Code, from January 1, 19_22_
to December 31, 19_2L
BE IT FURTHER RESOLVED, that only those streets, highways, and operations as listed and described on the ap-
proved Municipal Estimate of Maintenance Costs, including supplemental or revised estimates approved in connec-
tion with this resolution; are eligible for maintenance with Motor Fuel Tax funds during the period as specified above.
BE IT FURTHER RESOLVED, that the Clerk shall, as soon as practicable after the close of the period as given
above, submit to the 'Department of Transportation, on forms furnished by said Department, a certified statement
showing expenditures from and balances remaining in the account(s) for this period; and
BE .74 RESOLVED, that the Clerk shall immediately transmit two certified conies of this resolution
to the district office of the Department of Transportation, at Schaumburg , Illinois.

Special Provisions
The following Special Provisions supplement the "Standard Specifications for the Road
and Bridge Construction," adopted J a 11 (AO. (L,1 I G�r?-
the "Supplemental Specifications and Recurring Special Provisions," adopted
JCAn (An, r(.1 I , I of 01- (as indicated on the check sheet included herein),
the "Standard dpecifications for Traffic Control Items," adopted
and the latest edition of the "Illinois Manual on Uniform Traffic Control Devices for
Streets and Highways" in effect on the date of invitation for bids. These special
provisions included herein apply to and govern the proposed improvement designated
as Section 97 -ccc%o -a and in case of conflict with any part or parts of said
specifications, said special provisions shall take precedent and shall govern.

SPECIAL PROVISION
FOR
COOPERATION WITH UTILITIES
This special provision amends the provisions of Articles 102.05 and 105.07 of the Standard
Specifications for Road and Bridge Construction, adopted by the Department of
Transportation.
102.05 Examination of Plans, Specifications, Special Provisions and Site of
Worts. Add the following paragraph after the second paragraph of this
Article:
"When the plans or special provisions include information pertaining to the location of
underground utility facilities, such information represents only the opinion of the Department as
to the location of such utilities and is only included for the convenience of the bidder. The
Department assumes no responsibility in respect to the sufficiency or the accuracy of the
information
shown on the plans relative to the location of the underground utility facilities. ft shall be the
Contractor's responsibility to determine the actual location of all such facilities. He shall also
obtain from the respective utility companies detailed information relative to the location of their
facilities and the working schedules of the utility companies for removing or adjusting them."
105.07 Cooperation with Utilities. Revise this Article to read:
"105.07 Utility Facilities. Utilities which are within the limits of the proposed construction are to
be moved or removed at no cost to the Contractor except as otherwise provided for in the
special provisions or as noted in the plans.
(a) For the purpose of this Article, limits of proposed construction for
utilities extending in the same longitudinal direction as the roadway,
shall be defined as follows:
(1) The horizontal limits shall be a plane, outside of, parallel to, and .6 meter (2
feet) distant at right angles from the plan or revised slope limits and the slope
limits extended vertically above the point of intersection of the slope limits and
the original cross - section surface.
In cases where the limits of excavation for structures are not shown on the
plans, the horizontal limits shall be a vertical plane 1.2 meter (4 feet) outside the
edges of structure footings or the structure where no footings are required.
(2) The upper vertical limits shall be the regulations goveming the roadbed clearance for the
specific utility involved.
(3) The lower vertical limits shall be the limits of excavation.
(b) For the purpose of this Article, limits of proposed construction for utilities crossing the
roadway in a generally transverse direction shall be defined as follows:

Specifications "); the latest edition of the "Illinois Manual on Uniform Traffic Control
Devices for Streets and Highways" in effect on the date of invitation for bids; and the
"Supplemental Specifications and Recurring Special Provisions: indicated on the Chec
Sheet included herein; all of which apply to and govern the construction k

Program, Section 97- 00000 -01 -6M in Cook County, and in case of conflict with any part
or parts of said Specifications, the Special Provisions shall take precedence and shall p

Eureka Avenue begins at a point approximately 33 feet south of the centerline of Peiffer
Avenue and ends in a northerly and easterly direction to a point approximately 20 feet ea
of the centerline of Walter Avenue, a distance approximately 1970 feet. st
Oak Lane /Court begins at a point approximately 20 feet east of the centerline of Eureka
Avenue and extends in a northerly and westerly direction to a point approximately 20 feet of the centerline of Eureka Avenue, a distance of approximately 1055 feet. t
Hillview Court begins at a point approximately 20 feet east of the centerline of Hillview
Drive and extends in an easterly direction, a distance of approximately 435 feet.
Description of Proiect
This project is a roadway resurfacing project and the work to be performed under this
contract shall consist of curb repairs, full depth pavement removal and patching,
bituminous surface course removal, bituminous surface course placement, sidewalk re air
and watermain extension to be performed at the locations directed by the Engineer. p
Construction Schedule
Prior to commencing construction operations, the Contractor or his authorized
representative shall meet with the Village Engineer of the Village of Lemont or her
representative and present, in writing, his proposed construction schedule. Said schedule

shall contain such information as the Village Engineer deems necessary, particularly in
regard to location and date of starting construction, location of off -site disposal areas and
access routes be used, and when various phases of the work (including restoration) will be
completed.
The schedule shall demonstrate that the Contractor can complete the project with a
guaranteed maximum of 35 working days for all work on Eureka Avenue, Oak
Lane /Court and Hillview Court. To insure compliance with the schedule, the contractor
shall be required with each invoice, to update the schedule showing work complete and a
revised schedule to complete.
Working Days
Except for such work as may be required to properly maintain lights and barricades, no
work will be permitted on Sundays or legal holidays, and on weekdays between 7:00 a.m.
and 7:00 p.m. On Saturdays, no work shall begin before 8:00 a.m. without specific
permission of the Engineer. Exceptions will be granted from this work with special needs
that can be performed without adversely affecting the residents.
Waste Sites
All waste material shall be hauled to a site of the Contractor feets choice. Cost of disposal
of same will be included in the contract cost and not paid by the Village as a Separate
item.
Construction Layout
Line and grade stakes will be provided by the Contractor and approved by the Engineer.
Existing Utilities
The Contractor will be responsible for coordinating with the Engineer and the utilities in a
timely manner to insure that no delays are incurred. Any Village owned utilities and other
personal property damaged by the Contractor shall be replaced in kind or better at no
extra compensation.
With regard to the existence or location of a buried utility, the Contractor shall call the
Joint Locating Utility Locating Information for Excavators (J.U.L.I.E.) toll free at 1 -800-
892 -0123 before starting excavation. Allow 48 hours for other than emergency assistance.
Maintenance of Roadways
Beginning on the date that the Contractor begins work on this project, he shall assume
responsibility for the normal maintenance (excluding snow removal) of all existing

roadways within the limits of the improvement. This normal maintenance shall include all
repair work deemed necessary by the Engineer. Traffic control and protection for this
work will be provided by the Contractor as required by the Engineer. This work shall also
include keeping the streets in the project area and adjoining streets free of mud and debris.
If directed by the Engineer to do so because mud and debris, the Contractor shall sweep
the streets at no additional cost. Roadway maintenance will not be paid for separately but
shall be considered incidental to the Traffic Control and Protection Standards.
Conduct of Construction Operations
Cooperation shall be extended by the Contractor to notify landowners as to when direct
vehicular access to their property will be curtailed an the approximate length of time of
such curtailment. Notice shall also be given when utility services will be interrupted.
Written and/or vocal notification will be given all affected properties 24 hours to access
curtailment. THE CONTRACTOR shall conduct his operations so that pedestrian access
to existing properties is maintained.
Heavy construction vehicles will not be permitted to cross over curbs or use private
driveways for turnarounds. Any damage to parkways, curbs and driveways caused by the
Contractor's operations will be repaired at his expense.
Construction Safety
The Contractor shall take all precautions necessary to protect the general public and his
workmen from hazardous locations that might occur within the limits of the improvement.
All trench openings and other construction openings extending below the pavement
subgrade shall be fenced off with an adequately supported 4 foot high snow fence at all
times when actual construction is not in progress at the opening location.
The duty of the Engineer to inspect all work done and materials furnished and to suspend
any work that is being improperly performed does not include review or approval of the
adequacy of the Contractor's safety measures, methods and bracing of excavating trenches
in, on or near the construction site.
Use of Fire Hydrants
No use of fire hydrants shall be permitted by the Village.
Water Distribution System Specifications
The applicable articles of the Standard Specifications for water and sewer main
construction in Illinois shall govern the construction work associated with water system
appurtenances listed elsewhere in these Special Provisions.

Tree Protection
The Contractor shall exercise reasonable care in order to protect all existing trees within
the project limits. Protection of trees shall include augering or root sawing a minimum of
10 feet in close proximity to trees. Any tree damaged in the course of the work shall be
properly pruned and painted with an approved commercial tree dressing. Cost of the
augering, root sawing, protection and repair shall be incidental to and included in the
contract unit prices for various construction items that are applicable.
Bracing and Trench Box
It shall be the Contractor's responsibility to protect open cut trenches as may be required
by state or federal law, as may be necessary to protect life, property or the work. Trenches
in pavements or in close proximity to improved streets or roadways shall be braced or
shall use trench box in a substantial and effective manner. Bracing may be removed after
backfilling has been completed to such elevation as to permit its safe removal. Bracing left
in place must be removed for a distance of three feet below the established street grade.
The cost of furnishing, placing and removing trench box and bracing shall be incidental to
construction, and included in the contract unit price for the work being done.
Protection of Utilities
In excavating trenches and laying pipe, all existing utilities, including water pipes and
services, sewer pipes and services, gas pipes and services, electric or telephone
transmission pole lines, cable or conduits, shall be protected, supported, maintained in
services and restored to the condition in which they were found, all at no extra
remuneration. Where any utility facility, including service connections, is endangered or
damaged by the work, the utility management shall be notified by the Contractor, and the
Contractor shall cooperate with the utility and pay the cost of protection and repair, if
damaged. The Engineer has endeavored to locate subsurface obstructions from field
surveys and available records, and known structures are shown on the plans or notice
given of their presence. While the work was carefully done, the elevations of sanitary and
storm sewers have been obtained from the field surveys; and, where possible, elevations
are shown on the plans. Wherever the Contractor seems it necessary to determine the
exact location of existing pipe, valves or other underground structures, the Contractor
may make any examinations that he may determine desirable in advance of the work. No
added compensation will be paid therefore.
Bituminous Surface Removal (Variable Depth)
This work shall consist of removing the existing bituminous surface at varying depths to
the limits directed by the Engineer, in accordance with the applicable portions of Section
440 of the Standard Specifications.

The depth of removal will vary between 0 inches to 11/2 inches. The removal shall be done
by cold milling. The machine used for the surface removal shall be a milling machine
meeting the requirements of Article 440.07 of the Standard Specifications.
Bituminous Surface Removal (Variable Depth) will be measured in accordance with
Article 440.07 of the Standard Specifications.
The removal shall vary from 11/2 inches at the gutter flag and taper to 0 inches at a point
typically 6 feet from the gutter flag.
This work shall be paid for at the contract unit price per square yard for BITUMINOUS
SURFACE REMOVAL (Variable Depth).
Combination Curb and Gutter Removal and Replacement
This work consists of removing and disposing of the existing curb, gutter or curb and
gutter which is broken, otherwise damaged, or deteriorated and non - existent, or required
for construction of ramped sidewalk for the handicapped, and the replacement with new
curb, gutter or curb and gutter of a type similar to that which is existing, unless otherwise
directed by the Engineer, in accordance with the applicable portions of Sections 440 and
606 of the Standards Specifications and as directed by the Engineer.
The Contractor shall machine -saw a perpendicular clean joint between that portion of the
curb, gutter or curb and gutter to be removed and that which is to remain in place. If the
Contractor removes or damages the existing curb, gutter or curb and gutter outside the
limits designated by the Engineer for removal and replacement, he will be required to
remove and replace that portion at his own expense to the satisfaction of the Engineer.
The cost of any additional material (aggregate subbase) and compaction necessary to
grade the area under the proposed curb due to unstable subbase material under the
existing curb and gutter, shall be considered incidental to the work.
Any ground adjacent to the curb, gutter or curb and gutter which is removed or disturbed
during the removal operations shall be restored to

CLASS D PATCHES TYPE III
Class D Patches
This work shall consist of the removal of the existing pavement and Bituminous Concrete
Replacement as directed by the Engineer. This work shall conform to the applicable
portions of Section 442 of the Standard Specifications.
This work shall be classified as follows:
Type III 15 sq. yds. or more
The patch limits shall be as marked by the Engineer. Patches shall be a minimum of 4 feet
in width, and equal to the abutting pavement in depth.
Construction Requirements:
The materials and the methods of performing the work shall conform to Sections 355 and
406 of the Standard Specifications with the following exceptions:
a) Traffic Control and Protection shall be in accordance with the Special Provision for
"Traffic Control and Protection ".
b) Pavement Removal. This work shall conform to Section 440 of the Standard
Specifications.
c) All holes, soft places and other defects in the subbase or subgrade shall be corrected
by the Contractor by removing the unstable material, adding subbase granular material
and by recompacting the subbbase or subgrade either by means of mechanical tamper
meeting the approval of the Engineer or a by a hand tamper weighing not less than
approximately 0.6 pound per square inch of tampering face. P.C. concrete or
compacted bituminous concrete patching material, as above specified, may also be
used for this work. If the subbase or subgrade is dusty or muddy or improperly
compacted, patching operations shall be suspended until the subbase or subgrade is in
a condition satisfactory to the Engineer.
d) Completion of Patch. The surface of the compacted patch shall be maintained in a
smooth condition. High spots shall be trimmed level with the pavement surface. If
depressions develop, they shall be filled with Surface Course, compacted with a
tandem or three -wheel roller. No additional compensation will be permitted for
maintaining smooth patches.
If the patched pavement is to be resurfaced on the same contract, minor depressions in the
patch surface may be filled and compacted as a part of the resurfacing operations.
Method of Measurement. Pavement removal and replacement will be measured in place
for Class D patches, and the area computed in square yards.

If additional pavement, subbase or subgrade material is removed due to negligence on the
part of the Contractor, the additional quantity of pavement removal and replacement or
subgrade material will not be measured for payment.
Basis of payment. This work will be paid for at the contract unit price per square yard for
CLASS D PATCHES TYPE III of the thickness specified.
The cost of any additional materials (aggregate or Bituminous Concrete) and compaction
necessary to grade the area under the proposed replacement due to unstable subbase
material under the existing pavement shall be considered incidental to the work.
Area Reflective Crack Control Treatment, System A
This work shall be performed in accordance with the applicable portions of Section 443
except that only System A will be permitted. Reflective crack control treatment shall be
constructed at the locations as directed by the Engineer.
This work shall be measured and paid for at the contract unit price per square yard for
AREA REFLECTIVE CRACK CONTROL TREATMENT, SYSTEM A which price
shall be payment for completing all work specified including Bituminous Materials (Prime
Coat). Only the area being treated shall be measured for payment.
Water Valves, 8"
Gate valves shall be CLOW, East Jordan AWWA R/W Resilient Wedge Valve or
approved equal. This work shall be performed in accordance with the applicable portions
of the Standard Specifications and as shown on the Plans. The provisions of Division IV,
Section 42, Article 42 -2.02 - Type and Mounting of the "Standard Specifications for
Water and Sewer Main Construction in Illinois" shall be deleted and replaced with the
following specification:
"Valves shall conform to the latest revision of AWWA C -509 Standard For
Resilient Seated Gate Valves."
"Wedge shall be constructed of ductile iron, fully encapsulated in synthetic rubber
except for guide and wedge nut areas."
"Wedge rubber shall be molded in place and bonded to the ductile iron portion,
and shall not be mechanically attached with screws, rivets or similar fasteners."
"Wedge shall seat against seating surfaces arranged symmetrically about the
centerline of the operating stem, so that seating is equally effective regardless of
direction of pressure unbalance across the wedge."

This work will be paid for at the contract unit price per each for WATER VALVES of the
size specified, which price will be payment in full or completing all work specified herein
including furnishing materials, installation, excavation, backfill, disposal and labor.
Ductile Iron Water Main
This item shall consist of all work and materials, including water main fittings, joints,
water main accessories, thrust blocks and tests, necessary to install water main in
accordance with the applicable portions of Section 561 of the Standard Specifications and
the project plans, except as modified herein.
The ductile iron pipe shall be U.S. Pipe - Tyton Joint Pipe conforming to ANSI
Specification A25.51 or AWWA C -151, Class 52 or approved equal. Wall thickness and
other properties shall be in accordance with applicable ANSI or AWWA designations.
Cement -mortar lining for water main and fittings shall conform to ANSI Specification
A21.4 or AWWA C -104.
Pipe laying condition to be Type 4, pipe is to be bedded in sand according to the
specifications to a depth of 6 inches underneath pipe, backfilled and compacted in 12
inches lifts to final grade with crushed granular material as directed by the Engineer, and

the details in the plans. The Contractor shall perform the activity according to
ANSI/AWWA C 150/A21.50.
Wherever water is encountered in the trench, it shall be removed during pipe laying and
jointing operations. Provisions shall be made to prevent floating of the pipe. Any
denaturing of the trenches shall be considered incidental. At no time shall trench water be
allowed to enter the water main.
All pipe shall be handled in such a manner as will prevent damage to the pipe or coating.
Accidental damage to pipe or coating shall be repaired to the satisfaction of the Engineer
or be removed from the job and methods of handling shall be corrected to prevent further
damage when called to the attention of the Contractor.
The pipe shall be inspected by the Engineer for defects while suspended above ground.
Dirt or other foreign material shall be prevented from entering the pipe or pipe joint during
handling or laying operations and any pipe or fitting that has been installed with dirt or
foreign material in it shall be removed, cleaned and relaid. At times when pipe laying is not
in progress, the open ends of the pipe shall be closed by a water tight plug or by other
means approved by the Engineer to ensure absolute cleanliness inside the pipe. All cutting
of existing water main pipes for the insertion of valves, trees or other fittings shall be
performed without damage to the pipe or pipe lining and so as to leave a smooth end at
right angles to the axis of the pipe. Any damaged water main shall be re -cut and replaced
by the Contractor at his sole expense.
Joint restraining glands are to be installed on all hydrants, tees, valves and bends according
to Village specifications.
Blocking to prevent movement of lines under pressure at bends, tees, caps, valves and
hydrants shall consist of Portland Cement Concrete at least 12 inches thick, placed
between solid ground and the fittings, and anchored in such a manner that the pipe and
fitting joints will be accessible for repairs. All bends of 11 'A degrees or greater, and all
tees and plugs, shall be thrust- protected to prevent movement of the lines under pressure.
Where conditions prevent the use of concrete thrust blocks, tied joints of a type approved
by the Engineer shall be used.
The work will be paid for at the contract unit price per foot for DUCTILE IRON WATER
MAIN, of the size specified, measured in place from valve to valve along the centerline of
the pipe. Laterals shall be measured from the centerline of the cross main. This price shall
include the cost of all pipe, joint materials, hydrostatic tests, disinfecting of the water
main, excavation, thrust blocking, retainer glands, restoration of topsoil and seeding and
all watermain fittings. Reducers shall be paid for as larger size pipe. This item shall also

include any and all items such as corporation stops (for testing), water, pumps, gauges,
meters and laboratory test costs.
Tapping Valves and Sleeves, 8"
This work shall consist of the installation of a tapping sleeve, tapping valve on the existing
water main and tapping the existing 8 inch water main at the locations directed by the
Engineer.
The tapping sleeve shall be the size indicated by the Engineer. The sleeve shall have
mechanical fittings to secure the sleeve to the existing water main. The tapping sleeve shall
have a flanged connection to the tapping valve. The 8 inch tapping valve shall be of
resilient wedge gate valve type with a flanged connection to the sleeve and a mechanical
joint to accept the reducer. Valves shall conform to the water valves specifications.
The existing water main shall be uncovered and exposed to allow calipering of the pipe in
advance of the pressure connection.
Sufficient length of main shall be exposed to allow for operation of the tapping machinery.
The main shall be supported on concrete pedestals, at sufficient intervals to properly carry
its own weight, plus the weight of the tapping machinery and fitting. Any damage to the
main due to improper or insufficient supports shall be repaired at the Contractor's
expense.
After the tapping saddle or tee has been mounted on the main the tapping valve shall be
bolted to the outlet flange, making a pressure tight connection.
The tapping machine, by means of a special adapter, shall then be bolted to the outlet
flange of the tapping valve, also making a pressure tight connection. After the tapping
machine is in place, the installation shall be pressure tested at operating pressure plus 50 %,
to ensure the integrity of the installation. Water under pressure can be introduced through
a port in the tapping machine. The tapping machine and the fitting shall be externally
supported, so that no additional weight is placed upon the main.
The tapping valve shall then be opened, allowing the shell cutter -pilot drill assembly to
advance through the valve body until contact is made with the wall of the main. With the
tapping machine's feed set, power shall be supplied, starting rotation of the cutter -pilot
drill assembly.
The minimum diameter cut permitted shall be specified by the Design Engineer. For
pressure taps through 12 inch diameter the minimum diameter shall be 1/2 inch less than the
nominal diameter of the pipe to be attached. For 14 inch through 20 inch installations, the
minimum diameter shall be 11/2 inches less; for larger taps the allowable minimum diameter
shall be 2 inches - 3 inches less than the nominal diameter of the pipe being attached.

When the pilot drill penetrates the wall of the main, the nozzle, valve body, and tapping
machine will be filled with water. The cut shall be continued for a sufficient period of time
after this indication to allow the coupon to be completely severed from the wall of the
main.
The coupon shall be retained on the pilot drill by means of latches, spring detents, wire
clips or threads on the pilot drill; depending upon the make of the tapping machine. As the
boring bar is retracted the coupon, pilot drill and shell cutter return back into the tapping
machine adapter.
At this time the tapping valve shall be closed, sealing the main. The tapping machine shall
be removed and the valve shall be opened to flush any foreign material.
This work shall be paid for at the contract unit price per each for TAPPING VALVES
AND SLEEVES 8 ", which price shall include all labor and materials to complete this
work as specified. Trench backfill will be paid for as specified in these special provisions
and the Standard Specifications.
Inserting Valves
This work shall consist of the installation of inserting valve and a jacking sleeve on the
existing water main without interruption of flow at the locations directed by the Engineer.
The inserting valves and jacking sleeve shall be the size indicated by the Engineer. The
sleeve shall have mechanical fittings to secure the sleeve to the existing water main. The
inserting valve shall be a double disc valve type and the valves shall conform to the water
valves specifications.
The existing water main shall be uncovered and exposed to allow calipering of the pipe in
advance of the pressure connection.
Sufficient length of main shall be exposed to allow for operation of the tapping machinery.
The main shall be supported on concrete pedestals at sufficient intervals to properly carry
its own weight, plus the weight of the tapping machinery and fitting. Any damage to the
main due to improper or insufficient supports shall be repaired at the Contractor's
expense.
After the jacking sleeve and cutter assembly has been mounted on the main the lower
casting and slide gate shall be bolted around the assembly, making a pressure tight
connection.
The tapping machine, housed in a temporary bonnet shall then be bolted to the lower
casting, also making a pressure tight connection. After the tapping machine is in place, the
installation shall be pressure tested at operating pressure plus 50 %, to insure the integrity

of the installation. Water under pressure can be introduced through a port in the tapping
machine. The tapping machine and the fitting shall be externally supported, so that no
additional weight is placed upon the main.
The slide gate shall then be opened, allowing the tapping machinery to make contact with
the cutting assembly on the main. With the tapping machine's feed set, power shall be
supplied, starting rotation of the cutter assembly.
When the cutting is complete, the cutter assembly and the removed piece of pipe shall be
retracted into the temporary bonnet and the slide gate shall be closed. The inserting valve
shall then be placed into the temporary bonnet and the pressure tight connection shall be
reestablished. The slide gate shall then be opened and the inserting valve shall be lowered
into its final position and secured. The jacking sleeve shall then be maneuvered into
position by means of a threaded rod assembly extending through the lower casting. After
the insertion valve and sleeve are secured the temporary tapping chamber shall be
removed. The final assembly shall be tightened and checked for leaks.
This work shall be paid for at the contract unit price per each for INSERTING VALVES
of the size specified, which price shall include all labor and materials to complete the work
as specified. Trench backfill will be paid for as specified in these special provisions and the
Standard Specifications.
Fire Hydrant with Auxiliary Valve and Valve Box
The minimum requirements for the design, materials and workmanship of all fire hydrants
shall conform to applicable portions of Section 564 of the Standard Specifications.
All hydrants shall be East Jordan Iron Works Model 5BR Fire Hydrant, Clow Medallion
or approved equal. The hydrant shall be equipped with a 51/4 inch valve opening. The
hydrant shall also be equipped with two 2A inch hose nozzles and one 41/4 inch pumper
nozzle. All fire hydrants shall be equipped with an auxiliary valve and cast iron valve box.
The auxiliary valve shall be a 6 inch ductile iron water pipe as specified under Ductile Iron
Water Main, 6 inch, in the Special Provisions. The valve boxes shall be of the adjustable
type, shall be set at finished grade, and shall have the valve box covers stamped "Water."
A minimum of 1 cubic yard of course aggregate, CA -1, shall be placed at and around the
base of the hydrant to insure proper drainage of the hydrant after use. The hydrant shall be
set on a concrete block to insure firm bearing for the hydrant base.
This item shall be paid for at the contract unit price per each for FIRE HYDRANT WITH
AUXILIARY VALVE AND VALVE BOX, which price shall include the cost of all labor,
materials and equipment necessary to install the fire hydrant with auxiliary valve and line
extension in the plans, sewer and water specifications and to the satisfaction of the
Engineer.

Water Service Line
Corporation Stops
The work shall consist of constructing water service line and corporation stops of the
required material and inside diameter in accordance with Section 562 of the Standard
Specifications, the details in the plans and as modified herein.
Water service line shall be type K copper tube. Corporation stops shall be Muller H-
15000, Ford F -600, or approved equal.
This work shall be performed after the proposed water main has been pressure tested,
disinfected and approved by the Village's Public Works Director. The corporation stops
shall be installed prior to pressure testing the water main.
The Contractor shall adjust the maintenance of traffic as required to traverse travel lanes.
Trenches shall be backfilled and paid for as trench backfill form 6 inches above the water
line to the proposed subgrade elevation. The backfill of the balance of the trench to the
existing pavement elevation shall be paid for or as "Aggregate for Temporary Access."
Bedding material up to 6 inches above the water service line shall be considered incidental
to the unit price per foot of WATER SERVICE LINE. Pavement restoration shall be paid
for separately.
This work shall be paid for at the contract unit price per foot for WATER SERVICE
LINE of the size specified and measured in place horizontally and CORPORATION
STOPS per EACH. This price shall include the cost of all pipe, joint material disinfection,
excavation, bedding material, blocking and fittings.
Valve Boxes to be Adjusted
This item shall consist of adjusting Valve Boxes to match the proposed surface grade, or
as otherwise directed by the Engineer. In order to make the necessary adjustments, the
Contractor may have to provide either slide -type or screw -type extension for the existing
facility. It shall be the responsibility of the Contractor to ascertain the existing facility, and
the necessary extension piece required to perform the adjustment. The installation of the
extension pieces or the proper manipulation of existing slide or screw type devices will be
the only adjustment allowed, and the use of physical force to raise or lower the existing
Valve Boxes will not be permitted.
This item will be paid for at the contract unit price for VALVE BOXES TO BE
ADJUSTED, which price shall be considered full compensation for all labor, equipment
and materials necessary to perform the work to the satisfaction of the Engineer.

Domestic Water Services Boxes
The Contractor shall provide either slide -type or screw -type extension and shall provide a
Mueller H -15154 curb stop or approved equals. This work shall be constructed in
accordance with the specifications given by the Engineer.
These items will be paid for at the contract unit price each for DOMESTIC WATER
SERVICE BOXES which price shall be considered full compensation for all labor,
equipment and materials necessary to perform the work to the satisfaction of the Engineer.
Valve Boxes, 8"
This item shall consist of furnishing and installing Valve Boxes. The Contractor shall
provide a screw -type extension for the valve boxes. All lids furnished shall have the word
"Water" cast into the lid. This work shall be constructed in accordance with the details
given by the Engineer.
This item will be paid for at the contract unit price as VALVE BOXES of the size
specified, which price shall be considered full compensation for all labor, equipment and
materials necessary to perform the work to the satisfaction of the Engineer.
Traffic Control Plan
Traffic Control shall be in accordance with the applicable sections of the Standard
Specifications, the Supplemental Specifications, the "Illinois Manual on Uniform Traffic
Control Devices for Streets and Highways," any special details and Highway Standards
contained in the plans, the Traffic Specifications and the Special Provisions contained
herein.
Special attention is called to Articles 107.09 and 107.14 of the Standard Specifications
and the following Highway Standards, Details, Recurring Special Provisions and Special
Provisions contained herein, relating to traffic control.
The Contractor shall contact the Village of Lemont's Engineering Department at least 72
hours in advance of beginning work.
At the preconstruction meeting, the Contractor shall furnish the name of the individual in
his direct employ who is to be responsible for the installation and maintenance of the
traffic control for this project. If the actual installation and maintenance are to be
accomplished by a subcontractor, consent shall be requested of the Engineer at the time of
the preconstruction meeting in accordance with Article 108.01 of the Standard
Specifications. This shall not relieve the Contractor of the requirement to have a
responsible individual in his direct employ supervising this work. The Village will provide
the Contractor the name of its representative who will be responsible for the

administration of the Traffic Control Plan. This work will be paid for at the contract lump
sum price for TRAFFIC CONTROL and PROTECTION, which price shall include all
costs of furnishing, placing, maintaining, removing and disposing of the required traffic
control devices, including placing and removing temporary striping as specified herein and
the State Standards or as directed by the Engineer.
Aggregate for Temporary Access
This work shall consist of construction and maintenance of an aggregate surface course
for temporary patching and approaches as specified in Article 107.09 of the Standard
Specifications.
Aggregate surface shall be constructed in accordance with the applicable portions of
Section 402 of the Standard Specifications except that the equipment required for the
work will be as directed by the Engineer. The aggregate surface course shall be placed at
locations as directed by the Engineer.
Maintenance shall consist of placing and compacting additional aggregate of the same type
and gradation as the surface aggregate.
When the use of the temporary roads and approaches is discontinued, the surface
aggregate placed in its construction and maintenance shall be removed and utilized in the
permanent construction or otherwise disposed of as specified in Article 202.03.
This work will be paid for at the contract unit price per ton for AGGREGATE FOR
TEMPORARY ACCESS, which price shall include all costs of furnishing, placing,
maintaining, removing and disposing of aggregate used in the construction of temporary
roads, driveways and approaches.
Fire Hydrants To Be Removed
Domestic Water Services Boxes To Be Removed
Valve Boxes To Be Removed
This work shall be performed in accordance with the applicable portions of Sections 564
and 565 of the Standard Specifications in accordance with the plan notes and as directed
by the Engineer. Fire hydrants, valves and valve boxes removed shall become the property
of the Village of Lemont. Ends of watermains to be abandoned shall be plugged.

Sodding Special
This work shall consist of preparing the ground surface, fertilizing the areas to be sodded
as specified in the plans and furnishing and placing the sod. All work shall be in
accordance with the applicable portions of Section 252 of the Standard Specifications.
The maximum pay width shall be one and a half (11/2') feet unless specifically directed
otherwise directed by the Engineer.
180 pounds of fertilizer nutrients per acre shall be applied at a 5:3:2 ratio as follows:

Watering shall be done as directed by the Engineer, in accordance with Article 252.08 and
252.09 of the Standard Specifications.
This work shall be measured in place and the area calculated in square yards and shall be
paid for at the contract unit price per SQUARE YARD for SODDING SPECIAL, which
price shall be full compensation for all labor, equipment and material to complete the work
as specified in these special provisions.
Special Provision for Construction Layout Stakes
The Contractor shall be required to furnish and place construction layout stakes for this
project. The Contractor will provide adequate reference points to the centerline of the
survey and bench marks as directed by the Engineer and listed herein. Any additional
control points set by the Contractor will be identified in the field to the Resident Engineer
and all field notes will be kept in the office of the Resident Engineer.
The Contractor shall provide field forces, equipment and materials to set all additional
stakes for this project, which are needed to establish offset stakes, reference points and
any other horizontal or vertical controls, including supplementary bench marks, necessary
to secure a correct layout of the work. Stakes for line and grade of pavement and /or curb
shall be set at sufficient station intervals (not to exceed 15 m. (50 ft.)) to assure substantial
conformance to plan line and grade. The Contractor will not be required to set additional
stakes to locate a utility line which is not included as a pay item in the contract nor to
determine property lines between private properties.
The Contractor shall be responsible for having the finished work conform to the lines,
grades, elevation and dimensions called for in the plans. Any inspection or checking of the
Contractor's layout by the Resident Engineer and he acceptance of all or any part of it
shall not relieve the Contractor of his/her responsibility to secure the proper dimensions,
grades and elevations of the several parts of the work. The Contractor shall exercise care

in the preservation of stakes and bench marks and shall have them reset at his /her expense
when any are damaged, lost, displaced or removed or otherwise obliterated.
Responsibility of the Contractor
(a) The Contractor will locate and reference the centerline of all road and streets.
The centerline of private entrances and short street intersection returns may not
be located or referenced by the Contractor.
Locating ad referencing the centerline of survey will consist of establishing and
referencing the control points of the centerline of surveys such as PC's, PT's
and as many POT's as are necessary to provide a line of sight.
(b) Bench marks will be established along the project outside of construction lines
not exceeding 300 m. (1000 ft.) intervals horizontally and 6 m. (20 ft.)
vertically.
(c) Stakes set for (a) and (b) above will be identified in the field to the Resident
Engineer.
(d) The Resident Engineer will make random checks of the Contractor's staking to
determine if the work is in conformance with the plans. Where the Contractor's
work will tie into work that is being or will be done by others, checks that will
be made to determine if the work is being or will be done by others, checks

overall grade and horizontal alignment.
(e) The Contractor will set all stakes for utility adjustments and for building fences
along the right of way of line.
(f) The Contractor will make all measurements and take all cross sections form
which the various pay items will be measured.
(g) Where the Contractor, in setting construction stakes, discovers discrepancies,
the Resident Engineer will check to determine their nature and make whatever
revisions are necessary in the plans, including the recross sectioning of the area
involved.
(h) The Contractor will accept responsibility for the accuracy of the initial control
points as provided herein.
(i) It is not the responsibility of the Resident Engineer, except as provided herein,
to check the correctness of the Contractor's stakes; any errors apparent will be
immediately called to the Contractor's attention and s(he) shall be required to

make the necessary correction before the stakes are used for construction
purposes.
(j) Where the plan quantities for excavation are to be used as the final pay
quantities, the Contractor will make sufficient checks to determine if the work
has been completed in conformance with the plan cross sections.
(k) The Contractor shall establish form the given survey points and bench marks all
the control points necessary to construct the individual project elements. S(he)
shall provide the Engineer adequate control in close proximity to each
individual element to allow adequate checking of construction operations. This
includes, but is not limited to, line and grade stakes, line and grade nails in
form work and /or filed or etched marks in substantially completed construction
work.
It is the Contractor's responsibility to tie in centerline control points in order to
preserve them during construction operations.
(1) At the completion of the grading operations, the Contractor shall set stakes at
30 m. (100 ft.) Station intervals along each profile grade line. These stakes will
be used for final cross sectioning by the Contractor.
(m) The Contractor shall locate the right of way points for the installation of right
of way markers. The Contractor shall set all line stakes for the construction of
fences by the Contractor.
(n) All work shall be in accordance with normally accepted self - checking surveying
practices. Field notes shall be kept in standard survey field notebooks and
those books shall become the property of the Village of Lemont at the
completion of the project. All notes should be neat, orderly and in accepted
form.
(o) For highway structure staking, the Contractor shall use diligent care and
appropriate accuracy. Points shall be positioned to allow reuse throughout the
construction process. Prior to the beginning of construction activities, all
structure centerlines and pier lines are to be established by the Contractor and
checked by the Engineer. The Contractor shall provide a detailed structure
layout drawing showing span dimensions, staking lines and offset distances.
Measurement and Payment
This item will be paid for at the contract lump sum price for CONSTRUCTION
LAYOUT.

Specifications for Road and Bridge Construction, adopted by the Cepartment of Transportation, during
the performance of this contract, the contractor for itself, its assignees, and successors in interest
(hereinafter referred to as the "contractor ") agrees as follows:

(6) That it will permit access to all relevant books, records, accounts and work sites by
personnel of the contracting agency and the Illinois Cepartment of Human Rights for purposes of
Investigation to ascertain compliance with the Illinois Human Rights Act and the Department's
Rules and Regulations.
(7) That it will include verbatim or by reference the provisions of this clause in every
.subcontract so that such provisions will be binding upon every such subcontractor. In the same
manner as with other provisions of this contract, the contractor will be liable for compliance
with applicable provisions of this clause by all its subcontractors; and further it will promptly
notify the contracting agency and the Illinois Department of Human Rights in the event any
subcontractor fails or refuses to comply therewith. In addition, the Contractor will not utilize
any Subcontractor declared by the Illinois Human Rights Commission to be ineligible for contracts
or Subcontracts with the State of Illinois or any of its political subdivisions or municipal
corporations.

SPECIAL PROVISION

WAGES OF EMPLOYEES ON PUBLIC WORKS
This contract may be subject to "AN ACT regulating wages of laborers,
mechanics and otner workers employed in any public works by the State, County,
City or any public body or any political subdivision or by anyone under
contract for public works ", approved June 26, 1941, as amended, except that
where a prevailing wage violates a Federal law, order, or ruling, the rate
conforming to the Federal law, order, or ruling shall govern.
If it is determined that the "prevailing rate of wages" will be used for this
contract, the following conditions will be required:
Not less than the prevailing rate of wages as found by the (public
body) or Department of Labor or determined by a court on review
shall be paid to all laborers, workers and mechanics performing work
under this contract. These prevailing rates of wages are included
in this contract.
The contractor and each subcontractor shall keep an accurate record
showing the names and occupations of all laborers, workers and
mechanics employed by them on this contract, and also showing the
actual hourly wages paid to each of such persons.
The submission by the contractor and•each subcontractor of payrolls,
or copies thereof, is not required. However, the contractor and
each subcontractor shall preserve their weekly payroll records for a
period of three years from the date of completion of this contract.
If the Department of Labor revised the prevailing rate of hourly
wages to be paid by the public body, the revised rate as provided by
the public body .shall apply to this contract.

Specifications for Road and Bridge Construction adopted January 1, 1997.
701.04 Change paragraph (b) subparagraph (2) to read:
"The contractor is required to conduct inspections of the worksite at a frequency that will allow for
the timely replacement of any traffic control device that has become displaced, worn or damaged. A
sufficient quantity of replacement devices, based on vulnerability to damage, shall be readily available
to meet this requirement."

SPECIAL PROVISION
FOR
TRAFFIC CONTROL DEFICIENCY DEDUCTION
Effective April 1, 1992
To ensure a prompt response to incidents involving the integrity of the work zone traffic
control devices, the Contractor shall provide a telephone number where a responsible
individual can be contacted on a 24- hour -a -day basis. When the Engineer is notified or
determines a deficiency exists, (s)he shall be the sole judge as to whether the
deficiency is an immediate safety hazard. The Contractor shall dispatch sufficient
resources within 2 hours of notification to make needed corrections of deficiencies that
constitute an immediate safety hazard. Other deficiencies shall be corrected within 12
hours. If the Contractor fails to restore the required traffic control and protection within
the time limits specified above, the Engineer will impose a daily monetary deduction for
each 24 -hour period (or portion thereof) the deficiency exists. This time period will
begin with the time of notification to the Contractor and end with the Resident
Engineer's acceptance of the corrections. For this project, the daily deduction will be
per day. In addition, if the contractor fails to respond, the Engineer may correct
the deficiencies and the cost thereof will be deducted from the monies due or which
may become due the Contractor. This corrective action will in no way relieve the
Contractor of his /her contractual requirements or responsibilities.
* The cost of the daily deduction will be calculated by dividing three percent of the
awarded contract price by the number of calendar days anticipated for this project. The
number of days anticipated for this project is =c" . This procedure is to be followed
regardless of whether the contract is based upon working days, contains a completion
date, or has an incentive /disincentive clause.
