R-12-02 02/25/02RESOLUTION is . pica- RESOLUTION AUTHORIZING APPROVAL OF ADDENDUM FOR ENGINEERING SERVICES WITH FRANK NOVOTNY & ASSOCIATES IN CONJUNCTION WITH THE FOURTH STREET RECONSTRUCTION PROJECT WHEREAS, the Village of Lemont requires engineering services related to additional work for the Fourth Street Reconstruction Project, and WHEREAS, the Village of Lemont requires engineering services related to Country Lane (Country Ct.) Reconstruction Project, and WHEREAS, the Village of Lemont has previously awarded a contract with the firm of Frank Novotny & Associates, Inc.; and WHEREAS, the Village and Frank Novotny & Associates, Inc. have agreed to the terms and conditions of the extra services to be provided. NOW, THEREFORE, BE IT RESOLVED, by the President and Board of Trustees that the revised agreement attached hereto as Exhibit A is hereby approved. REVISE: Page 1 of 8, Paragraph 1 to read as follows: THIS AGREEMENT, made and entered into this day of , 2002, by and between FRANK NOVOTNY & ASSOCIATES, INC., whose address is 825 Midway Drive, Willowbrook, Illinois, 60521, hereinafter called the "ENGINEER ", and the VILLAGE OF LEMONT, whose address is 418 Main Street, Lemont, Illinois 60439, hereinafter called the "OWNER" covers certain professional Engineering services in connection with the proposed improvement designated as FOURTH STREET PAVING: SOUTH OF COUNTRY LANE TO McCARTHY ROAD and which project consists of the reconstruction of Fourth Street, from south of Country Lane to McCarthy Road, to a twenty -seven foot (27') width, back to back of curbs in a forty foot (40') width right -of -way, including pavement removal and replacement, combination concrete curb and gutter, on -site and outfall storm sewer, coordination of overhead utility relocation, water main improvements, appurtenances, revisions to current design to accommodate new pavement width and new alignment, and the drawing up of Plats of Easement for the outfall storm sewer, a net length of approximately 2,100 feet. Also included is the proposed improvement designated as COUNTRY LANE AND COUNTRY COURT PAVING & WATER MAIN IMPROVEMENTS, which scope of construction work consists of the reconstruction of Country Lane, from Fourth Street to Kromray Road, and Country Court, north of Country Lane, to a twenty -seven foot (27') width, back -to -back of curbs in a sixty -six foot (66') width right -of -way, including pavement removal and replacement, combination concrete curb and gutter, on -site storm sewer, water main, and appurtenances, a net length of approximately 1,250 feet.
