RESOLUTION APPOINTING BENJAMIN P. WEHMEIER AND GEORGE J. SCHAFER
AS DELEGATES TO THE SOUTHWEST AGENCY FOR HEALTH MANAGEMENT
WHEREAS, the Village of Lemont adopted the Contract and By -Laws of the Southwest Agency
for Health Management (SWARM) by Ordinance and thereby became a member of said cooperative; and
WHEREAS, the Village of Lemont seeks to appoint delegates to represent the Village of Lemont
in said cooperative.
NOW, THEREFORE, BE IT RESOLVED by the CORPORATE AUTHORITIES, of the
Village of Lemont, Cook, Will and DuPage Counties, Illinois, that Benjamin P. Wehmeier, Administrator
of the Village of Lemont is hereby appointed to represent the Village of Lemont on the Board of Directors
of said Southwest Agency for Health Management (SWAHM); and
BE IT FURTHER RESOLVED that George J. Schafer, Assistant Village Administrator of the
Village of Lemont is hereby appointed as the alternate representative to serve if Benjamin P. Wehmeier -
Delegate, is unable to carry out his aforesaid duties as the representative of the Village of Lemont to said
cooperative.

VILLAGE OF LEMONT, COOK, WILL AND DU PAGE COUNTIES, IL on this 25th day of
