attached hereto is a true and correct copy of the Resolution 2-08-o 4- now on file in my
office entitled:
A RESOLUTION EXPRESSING VILLAGE'S DESIRE TO
PARTICIPATE IN PUBLIC WORKS PROJECTS PURSUANT TO THE
COOK COUNTY COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM
which Resolution was passed by the Board of Trustees of the Village of Lemont at a
meeting held on the 9th day of February, 2004, at which meeting a quorum was present,
and approved by the Mayor of the Village of Lemont on the 9th day of February, 2004.
I FURTHER CERTIFY that a fully executed original copy of said Resolution was
duly filed in the office of the Clerk on February 9, 2004 and that attached hereto is a
copy of said Resolution. That the original Resolution is entrusted to my care for
safekeeping and that I am the lawful keeper of same.

RESOLUTION

A RESOLUTION EXPRESSING VILLAGES DESIRE TO
PARTICIPATE IN PUBLIC WORKS PROJECTS PURSUANT TO THE
COOK COUNTY COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM
WHEREAS, Cook County, as the Grantee of Federal Funds, has instituted a
program know as the Community Development Block Grant Program in order to
implement the distribution of said funds through the United States Department of
Housing and Urban Development; and,
WHEREAS, application for funds for the 2004 Program Year commencing
October 1, 2004, and ending September 30, 2005, are presently being accepted by
Cook County as Grantee; and,
WHEREAS, the Village of Lemont is desirous of participating for the specific
purpose of the following Public Works projects for Program Year 2004;
NOW, THEREFORE BE IT RESOLVED by the Mayor and Board of Trustees of
the Village of Lemont, Illinois as follows:
Section 1: That a Request is hereby made to the County of Cook, Illinois for
Community Development Block Grant ( "CDBG ") funds for Program Year 2004 in the
amount of $405,600.00 for the following projects:
PRIORITY NO. 1
Illinois Street Water Main Replacement
State Street to Stephen Street
The replacement of 900 feet of existing 3 -inch water main with a
new 10 -inch ductile iron water main..
(Area 8240 -01, BG -3, 56.2 %) $ 153,000.00
Engineering, Material Testing & Soil Borings $ 20,000.00
Sub -Total $ 173,000.00
Village of Lemont Share (20 %) $ 34,600.00
CDBG Funds requested as identified in the
Village's CDBG Program Year 2004 Application $ 138.400.00
PRIORITY NO. 2
Illinois Street Water Main Replacement
Stephen Street to Grant Street
The replacement of 1,070 feet of exiting 6 -inch diameter
water main with a new 10 -inch ductile iron water main.

Engineering, Material Testing & Soil Borings $ 23,100.00

McCarthy Road and Stephen Street Water Main Replacement
The replacement of 300 feet of 6 -inch water main on
Stephen Street, from Porter Street to McCarthy Road, and
350 feet of 4 -inch water main on McCarthy Road, from
Holmes Street to Grant Street, with a new 10 -inch ductile
iron water main.

Village's CDBG Program Year 2004 Application $ 103.200.00
Section 2: That the Village of Lemont, Illinois has developed a plan which addresses
development plans to improve the local community, its housing, its infrastructure, and its
local tax base, which plan was previously adopted by the Village of Lemont, Illinois.
Section 3: That the Mayor of the Village of Lemont is hereby authorized to certify that
local funds which have been identified as supporting its projects as set out within its
application will be made available upon the approval for the projects by the County of
Cook, Illinois, or pro -rated share thereof.
Section 4: That the Village Mayor and Village Clerk are hereby authorized to sign the
application and various forms contained herein; make all required submissions; and do
all things necessary to make application for the funds requested in Section 1 of this
Resolution, a copy of which application is on file with the Village Clerk.
Section 5: That the Village has published a notice of two (2) separate Public
Hearings held on January 27, 2004, and February 2, 2004, respectively, to elicit
comments from the public and to describe the program to the public. The Village will
comply with any and all other requirements of notice, reporting, maintenance, or any
other obligation as may be set forth under existing rules and regulations of the
Community Development Block Grant Program.
Section 6: This Resolution shall be in full force and effect from and after its passage
and approval as provided by Law.
